When you report an incident, you'll receive an acknowledgement. If necessary, we'll contract you to ask for additional information. The person dealing with your report, such as an occupational health and safety coordinator, will decide whether further investigation is necessary.

Privacy of reported incidents

When reporting, registering and handling (near-)incidents, the UvA only processes the data necessary for:

• Quality management and improving our organisation
• Integrated safety management and safeguarding health and safety

Incidents are handled by a select group of designated staff. The questions in the form are limited to what is strictly necessary; you can add additional information yourself only where needed. Data is processed confidentially, retained only for as long as necessary, and used to limit harm, prevent recurrence and comply with applicable laws and regulations.
